To be considered for Federal Pell Grant, Federal SEOG, Federal Perkins Loan,
Federal Work-Study, Federal Stafford Loan and/or PHEAA State Grant,
you must complete the following financial aid applications/forms:
- Free Application for Federal Student Aid (FAFSA) or Renewal
FAFSA Application
- If you are selected for verification, a Verification Form will
be sent to you to complete and attach appropriate documents and
submit to the Financial Aid Office.
- For a Federal Stafford Loan, a pre-certified Master Promissory
Note (MPN).

Required Forms
All the forms listed below are not required for all types
of financial aid.

Free Application
for Federal Student Aid (FAFSA)
First time applicants must complete an original
Free Application for Federal Student Aid. It should be received
by the processor on or before March 15th
(but not before January 1st) to be considered for one or
more of the financial aid awards listed above. Paper applications
are no longer available. You can apply online at FAFSA
on the Web (www.fafsa.ed.gov) to electronically send your FAFSA.

| Renewal
Application
Currently enrolled students who already have
information in the federal student aid files will receive
a postcard with his/her
PIN number (if he/she filed online). This forms will be
mailed in November and December. The Renewal Applications
must be completed by March 15th. Students
must complete the FAFSA by March
15th to be considered for all types of financial
aid. The FAFSA can be completed online at www.fafsa.ed.gov

| Millersville
University Verification Form
This form must be completed if you are selected
for verification and you want to be considered for a Federal
Pell Grant, Federal SEOG, Federal Perkins Loan, Federal
Stafford Loan and/or Federal Work-Study. The Verification
Form will be sent to students who have been selected to
complete the verification process. The Verification Form
and all the appropriate documents must be received in the
Financial Aid Office by May 15th in order to receive full
consideration for all types of aid. Failure to complete
the verification process will prohibit you from receiving
any federal financial aid for the academic year for which
you are applying.

| Federal
Stafford Loan MPN
First time borrowers at MU will automatically
be mailed an Master Promissory Note (MPN) from American
Education Services (AES, formerly known as PHEAA) after
the University has pre-certified his/her maximum Stafford
Loan eligibility. Students who borrowed a Stafford Loan
after July 2000 and prior to the current academic year,
will not be mailed an MPN. Any student who has not yet completed
an MPN (first-time borrowers), or who borrowed a Stafford
Loan prior to July 2000 will need to fill out a new MPN.
A student will be given the maximum Stafford loan eligibility,
unless the student returns the "Borrowing Intention Form"
(BIF) that is mailed with their award letter indicating
otherwise.

Note: The financial aid application
process is lengthy, and funds for most programs are limited.
Please complete the necessary
forms as soon after January 1 as possible. |
