Authorization to Release Information Form
For students to complete to allow us to speak with persons other than the student about his/her account.

Financial Aid Federal Appeal Form
This form is for students who have lost their financial aid due to not passing enough credits during the academic year (Satisfactory Academic Progress - SAP) - it is for Federal Aid ONLY (does not include the PA State Grant). The review for Satisfactory Academic Progress is done at the end of every spring semester. Appeal forms are generally due mid-June and if the form is not received by the due date, the appeal will not be reviewed until mid-September (after the start of the fall semester).

2007-2008 Loan Changes
If you are a current student (enrolled in courses for 2007-2008) and wish to change your loans for this current year (2007-2008), please contact the Office of Financial Aid at 717-872-3026.

The Loan Change form is for students who wish to reduce, cancel, or increase their loan amounts.
Please realize any request for an increase is pending verifcation of eligibility for the increase.
Please see our website on Federal Stafford Loans to review the limits for each year.

Part-Time (Less-than-Full-Time) Form (online submission)
This form is for any student who will not enroll as a full-time student during the fall or spring semester - this is an online form.
Full-time for undergraduate students is 12 credits.
Full-time for graduate students is 9 credits.

PLUS Dependency Status Form
This form is for students to complete if his/her parents have applied for the Federal PLUS loan and the student has not completed the FAFSA.

Summer Aid Application - online submission
This form is for students who wish to enroll in summer courses and use financial aid to help cover the cost of their courses with financial aid. Students must read the information regarding summer financial aid prior to completing the application.
