1.  Print Academic Minor Form

2. Fill out top section and get your advisor's signature

3. Fill out the "declare minor" section

4. Take form to Entrepreneurship Minor Director (Dr. Stephanie Schwartz, 133 Roddy) for signature Dr. Schwartz's office hours

5. Return completed form to Academic Advisement (second floor Lyle Hall)
