To be medically eligible to participate in athletics at Millersville University you must meet the following criteria:

All freshman and newcomers

Have an entrance physical on file with the Health Center as is required of all new students.

1. Go online at Sportsware link and update your relevent information.
2. Pass a comprehensive physical given by the Health Service. You may fill out your medical history form prior to your appointment in order to save time. The form can be downloaded here. You may choose to fax it to the Health Center at 717-871-2243.
3. Yellow card issued at the completion of the physical. It must be filled out and turned in to your respective equipment manager.

All returning athletes who have participated within the last year in MU athletics

1. Go online at Sportsware link and update your relevent information.
2. Make an appointment at the Health Center for a review of your health history and blood pressure check.
3. Yellow card issued at the completion of the physical. It must be filled out and turned in to your respective equipment manager.
