Grant Information/Request Form
If you are interested in applying for a grant and need assistance with funding searches or with other aspects of grant writing, please fill out and submit the following form. This will provide us with an opportunity to understand and research your needs before calling or scheduling a meeting.
Proposal Approval Form
Before writing a proposal, consult with the academic department chairperson or the office director to ensure support for, understanding of the obligations and reporting requirements of the project. Contact the Office of Government and Foundation Support at the start of the grant writing process to help with accounting and other matters. Once your proposal is complete, you must seek University approval before submitting it to the funding agency. University Approval can be a lengthy (10 working days or more) process and we ask that you budget enough time for approval. GRANTS CANNOT BE SUBMITTED UNTIL THIS FORM IS COMPLETED AND SIGNED BY ALL PARTIES.
Conflict of Interest Form
This form must be completed and attached the proposal. It should circulate with the Proposal Approval Form. GRANTS CANNOT BE SUBMITTED UNTIL THIS FORM IS COMPLETED AND SIGNED BY ALL PARTIES.

Institutional Research Board (IRB)
These forms are necessary if you plan to include any human subjects in your research.
Please visit the Millersville University Institutional Research Board (MUIRB) website, read the relevant federal regulations and MU Guidelines, and see the relevant FAQs for more information concerning the approval process and scheduling. Depending on the meeting schedule, Board approval may take a month or more - please budget your time accordingly
Institutional Animal Care and Use Committee (IACUC)
