Before downloading and submitting an application to complete any internship, Social Work field placement, or student teaching placement, you MUST refer to the specific process outline which lists the step by step guidelines in detail BEFORE starting an application.

Read the following directions before beginning your application packet.

1. Select your program's application (in Section A)
2. Review eligibility requirements (Section A, page one)
3. Reference the checklist (Section A, page two)
4. Print, sign and date the appropriate pages
5. Submit the application packet to the appropriate offices.

Note: This application is a "typeable" PDF document, which means that you must first save a blank copy to your computer and then use the PDF from your computer to complete your application. This will allow you to save your application each time changes are made.  Handwritten application packets will not be accepted.
