General Information Form:
This form is kept on file in the SWAT Office in Boyer and provides us with information regarding your employment.
Verification and Acknowledgement Form:
This form verifies that you read the SWAT handbook and accept the responsibilities of your assigned position.
IF YOU ARE NEW TO WORKING ON CAMPUS:
1-9 Form:
This form verifies your employment eligibility in the United States. BEFORE YOU BEGIN WORK, KATHY MULL MUST SEE TWO FORMS OF IDENTIFICATION from you (usually an original social security number and driver's license) to complete this form. Please fill out only part #1 of this form; Kathy will complete the rest.
W-4 Form:
This form is used for federal tax purposes. The home address indicated on the form should be your permanent residence (i.e. the residence your taxes will be filed under). Those persons entering EXEMPT in part #7 need to submit a W-4 form every year to remain exempt.
Student Payroll Card:
This card will stay on file with the Student Payroll office. The municipality for wage tax purposes should be your permanent residence (i.e. the residence your taxes will be filed under). This card also contains the information that the Student Payroll office will use to send W-2 statements and paychecks to (i.e. the LOCAL address indicated on the card), therefore, it is important to keep this information up to date with the Student Payroll office. Local address changes can be made via a phone call to Student Payroll, however, permanent address changes must be done by filling out a form available in the Student Payroll office in Dilworth.
Special Tax Information for all Employees:
If you were employed by the University in previous years and submitted a W-4 and claimed EXEMPT, you must fill out another W-4 form this tax year to remain EXEMPT.
Special Tax Information for Non-PA Residents:
If your permanent residence is out-of-state and is one of the following: Virginia (VA), West Virginia (WV), Maryland (MD), Ohio (OH), or Indiana (IN), there is a form you can fill out in the Student Payroll office so that you do not have to pay Pennsylvania state tax. Residents of New York (NY) and Delaware (DE) are required to pay Pennsylvania state tax. New Jersey (NJ) residents have state tax taken out in Pennsylvania, but are credited for it when filing New Jersey state tax.

- As any of the SWAT workers' knowledge and skill improves, he/she will be eligible for increased responsibilities and more complex projects. He/she will also be eligible for pay raises as skills improve and as he/she continues to serve on the team. Reviews will be held at the beginning of the Fall, Spring, and Summer semesters.
- SWAT workers are only paid for hours that they work and that are logged on the pay sheet by 5pm on the last Friday of each pay period.
- Instructional Designers should log the hours they work on projects and customer service on a weekly Project/Service Form. Be sure to completely fill in the form, to keep it updated, and to store it neatly in your folder. If an Instructional Designer cannot work at his/her scheduled time, he/she must leave a message for Al Unrath, prior to the scheduled time.
- Multimedia Technicians must complete a daily time report online at: http://www.millersville.edu/ ~wms/hours/ (userid and password will be handed out on the first day or work). If a Multimedia Technician cannot work at his/her scheduled time, he/she must leave a message for Stephen Gadsby, prior to the scheduled time.
- Web Technicians and Web Designers must complete a daily time report online at: http://www.millersville.edu/ ~wms/hours/ userid and password will be handed out on the first day or work). If a Web Technician cannot work at his/her scheduled time, he/she must leave a message for Ryan Griffith, prior to the scheduled time.
