Online Programs Student Forms

The Change of Delivery (COD) forms are to be used by students wishing to switch either from one of Millersville University's fully-online undergraduate programs to the face-to-face (F2F) format, or, more commonly, vice versa.

These forms are only to be used by those students either admitted (accepted) to or already enrolled in a program which has both a F2F and fully-online version (e.g., the RN-to-BSN Program).

With the exception of the RN-to-BSN Online COD Form (which can be filled out right on your computer or mobile device), these forms must be filled out by hand.  The completed form should then be submitted as an email attachment to OnlinePrograms@millersville.edu.

Change of delivery: F2F to Online (all programs except RN-to-BSN Online; must be filled out by hand)

This form can be used, for example, for switching from the F2F Social Work Program to the fully-online format.  This form must be filled out by hand and must be signed by both your academic advisor as well as your Department Chair.  RN-to-BSN students wishing to switch formats must use the BSN-specific COD form.

Change of delivery: Online to F2F (all programs except RN-to-BSN Online; must be filled out by hand)

This form can be used, for example, for switching from the Social Work Online Program to the F2F format.  This form must be filled out by hand and must be signed by your academic advisor, your Department Chair, as well as the Coordinator of the Online Program you wish to join.  RN-to-BSN students wishing to switch formats must use the BSN-specific COD form.

Change of delivery: F2F to Online (BSN) (RN-to-BSN Online only)

This form is to be used only for RN-to-BSN students wishing to join the online format.  It can be filled out right on your computer or mobile device.  Please be sure to fill out both pages in full.

Change of delivery: Online to F2F (BSN) (RN-to-BSN Online only)

This form is to be used only for RN-to-BSN students wishing to join the F2F format.  It can be filled out right on your computer or mobile device.

The following forms are to be used only by those students who are completing one of Millersville University's fully-online undergraduate programs.

Online Graduation Application (for students graduating from fully-online undergraduate programs)

This convenient form is filled out directly in your web browser, and no paper form is required to be filled out at all.  Once your graduation application has been submitted, you will be registered for GRAD 999 automatically (if you have not already done so yourself).

Online Certificate Completion Application (Undergraduate) (for students in fully-online undergraduate certificate programs only)

After completing this form, please submit it to the Office of Online Programs, preferably as an email attachment to OnlinePrograms@millersville.edu.
