Please complete the following information indicating the type of loan(s) you would like to request or change. Changes will be reflected on MAX after your request has been processed

Please be aware that if you cancel the first disbursement of your loan, the entire loan will be cancelled. If you have any questions, please contact your financial aid counselor.

Please note: Only the student may make changes to his/her Federal Stafford Loan or Graduate PLUS Loan, since it is a loan solely in the student’s name. Only the parent who borrowed the Federal PLUS loan may make changes to the Federal PLUS Loan. Only the student and/or co-signer may make changes to the Alternative Loan.
