Academic Major Form (print version only)
Use this form to change your major, your major advisor, or declare or drop a second major.
Academic Minor Form (print version only)
Use this form to declare or drop a minor.
Address Update and Emergency Contact Information (online version)
Use this online form to update your address and emergency contact information. Remember, without a correct address on file, you will not be able to receive important notices, including your bill. If you need to change your name, please provide the Registrar's Office with official documentation, e.g. photocopy of social security card, court decree, marriage certification.
Authorization to Release Information (print version only)
Completion of this form is necessary if you would like your financial aid information released to anyone other than
yourself (including your parents). It is valid for the student's academic career. If there is a change, a new form
should be completed and the old form will be void. The form should include anyone, including parents, to whom
the student would like financial aid information released. Please read the form carefully before completing it.
Clear Notification Form (online version)
Use this form to notify the Bursar's Office of your intent to attend MU for the current semester. If you
received a tuition bill and have either a zero balance or a (-) credit balance, complete this form to
"clear" your bill, avoid a late fee, and/or the possibility of having your schedule dropped.
Direct Deposit Form For Student Refunds (print version only)
Return this completed form to the Bursar's office to sign up for direct deposit of student refunds.
Exception to Graduation Requirements (print version only)
Use this form to request exceptions in the major, minor, general education or university academic policy requirements. Note: this needs to be done well in advance of your expected graduation date.
Federal Aid AuthorizationForm
(online version)
Grant the Bursar's office your permission to use Title IV funds to cover miscellaneous expenses charged
on your student account. If there is an excess of funds once the standard costs have been paid, your
permission is needed in order for federal funds to cover these additional charges.
Grade Report Requests (online version)
Use this form to request a paper grade report on a university form be mailed to your mailing address.

MIPP Brochure and Form(print version only)
The Millersville University Installment Payment Plan (MIPP) enables
participants to make regularly scheduled monthly installments out of current income. In addition, MIPP
allows participants to determine how much of tuition, fees, room and board they may want to pay in
installments. This feature affords our participants the ability to budget costs as best fits their
personal situation.
Part Time Enrollment Forms
for Financial Aid
Use the form(s) located here if you are an undergraduate or certification student taking less than 12 credits per
semester or a graduate student taking less than 9 credits per semester to prevent the delay of your financial aid
disbursements.
Permission (Academic
Department) to Enroll in a Restricted Class (print version only)
Use this form to receive permission from the faculty to enroll in a course that has restrictions; such as permission required,
prerequisites, limited to majors or class level (Jr., Sr., etc.). The completed form will be processed by the academic
department.
Permission for an Undergraduate Student to enroll in a Graduate Class (print version only)
An undergraduate student who wishes to take a graduate 500-level course must have this form completed by the department
offering the course. The completed form will be processed by the Registrar's Office.
Permission to Enroll in a Class that has a Time Conflict (print version only)
If you have two courses that have a time conflict and one of the faculty is willing to work with you about missed time,
please use this form. Register for the first course on MAX and bring this form to the Registrar's Office so you can be
registered for the second class.
NOTE: The overlap in classes should not exceed 15 minutes.
Transcript Requests - mail (print version only)
Use this form to request a transcript to be mailed to you or to a third party. Note: there is no fee. Allow 10 business days for processing.
Transfer of Credit (print version only)
Use this form to request authorization to pursue credits at a different institution and transfer the credits back to MU.

Use this form to withdraw, whether during or between semesters and regardless of reason. To return to MU, you will need to reapply.
