Graduate Student Forms Center

In an effort to increase the customer focus of our graduate student services, the Office of Graduate Studies and Research is pleased to provide on-line access to many of the common forms that graduate students need during their degree program. Hard copies of these forms are also available in the Office of Graduate Studies and Research, 2nd Floor, Lyle Hall. Please note that the following forms are being provided in .pdf format (Abode Acobrat), unless noted otherwise, for your use.

Forms dealing with degree progress and completion:

Application for Admission to Degree Candidacy (all master's programs) (.pdf format) - intended for students seeking to advance to degree candidacy. It is student's responsibility to initiate the candidacy review process by applying to their respective department within the required semester-hour limitation.

Application for Graduation (.pdf format) - intended for students working in the final semester of their master's program and seeking to be evaluated for graduation.

Change of Graduate Curriculum for Master's Degree Programs (.pdf format) - intended for students seeking to change from their current master's degree program to another master's degree program at MU. Students should note that additional information may be necessary in addition to this form.

Forms dealing with courses:

Request for MU Course Substitution (.pdf format) - intended for students seeking to substitute a required course in a graduate program with another graduate course at MU (from within their home program or from a different program).

Request for Transfer of Credit Back to MU (.pdf format) - intended for matriculating students seeking to take a course at another university to transfer back to their master's degree program at MU. Prior approval from the adviser, graduate coordinator, and dean of graduate studies and research is necessary.

Transcript Review of Graduate Courses Completed at Another Institution Prior to Admission at MU (.pdf format) - intended for students seeking to transfer credit for graduate courses taken at another regionally accredited institution prior to their MU admission. Please note that transfer credit will not be awarded for courses taken elsewhere that were specifically used in the completion of another degree program.

Transcript Review of Graduate Courses Completed at MU (.pdf format) - intended for students seeking to transfer credit for graduate courses completed at MU in programs (including nondegree status) other than the one they are planning to pursue.

Other forms for graduate students:

Change of Address/Emergency Contact Information Form (on-line format) - it is important for graduate students to keep the Office of Graduate Studies and the Registrar informed about changes to their temporary or permanent address and emergency contacts.

  • New off-campus cohort being planned for MED in Special Education
    Graduate Studies, in collaboraton with the School of Education, is promoting a new graduate cohort for the MED in Special Education to be delivered on-site in Harrisburg. Courses are being planning for delivery at the Dixon University Center and in an on-line format. Recruitment for the cohort has started and interested students should contact the Graduate Studies office at 872-3099.
  • New Graduate Student Orientation
    The next graduate studies orientation event for new graduate students (for spring admits) is scheduled for Monday, January 12, 3:00 to 6:00pm. For more information, click the link at the left under "For Current Students."
  • New Graduate programs in Emergency Management and Gifted Education
    The newly approved MS in Emergency Management (on-line) and M.Ed. in Gifted Education programs, both started in fall semester 2007, are currently inviting applications for the spring 2008 term.
