Request for IRB Protocol Number for Pending Research: (PDF format)
For researchers (faculty, staff, or students) that are planning to make application for grant funding from Millersville Univeristy or from outside agencies or foundations where an IRB Protocol Number is required. The researcher, if awarded the grant funding, will need to follow-up with a full Human Subjects Review Protocol, approved by the IRB, before implementing the research project.

Request for Extension of Previously Approved Protocol: (PDF format)
Previously approved protocols that require a time extension for completion of research must be reviewed by the IRB. Requests for extension should be made to the IRB six weeks prior to the protocol's expiration date.
