Eligibility

All registered students are eligible to use the Health Services. Spouses and dependents are not eligible. It is a REQUIREMENT for each incoming student to have a completed Health Evaluation Form on file in the Health Services. This includes a physical examination by your family physician, a current Mantoux (test for tuberculosis) and a complete immunization record.

Failure to comply with this requirement results in a student being placed on "Hold". This means that the student would be unable to register for future classes and would not receive grades for the current session until the Health Evaluation Form (PDF) is submitted.
