Housing Status Forms

This form is for financial aid purposes only and is used to ensure your financial aid and cost of attendance are accurate. This form is separate from any other type of housing form that may be requested by other departments.

  • If this form is requested of you under your financial aid requirements, please be aware that failure to complete it will prevent the processing and payment of your financial aid. 
  • This form should also be completed if the housing status listed for you is inaccurate. Failure to notify the Office of Financial Aid of changes to your housing may result in inaccurate financial aid awards.

While completing the Financial Aid Housing Form will serve as a notification to the Office of Financial Aid of changes in your housing or to update your local address for financial aid purposes, it will not update your information with any other department on campus.

  • Please make sure to notify the appropriate offices of housing changes and update any address changes on MAX and/or in your SAM portals.

2024-2025 housing Status 

This form is required for financial aid purposes and applies to students with enrollment during Summer 2024, Fall 2024, and/or Spring 2025 semesters. 

  • If this form is requested of you, please be aware that failure to complete it will prevent the processing and payment of your financial aid.
