New Student-Athlete Medical Clearance Requirements

Student-athletes who did not compete for Millersville University in the 2023-2024 academic year need the following items in order to be medically cleared:

  • Submitted sickle cell results
  • Complete Sportsware account
  • Physical from primary care provider within the past year. This form can be used for the exam.
  • Vaccination records with current tetanus vaccine
  • A baseline concussion assessment*
  • A pre-participation exam (PPE) completed by the Millersville University supervising team physician*
  • Mental health screen**

*The baseline concussion assessment and  pre-participation exam, and will be completed upon the student-athlete's pre-particpation exam date. 

**The mental health screen will be completed upon the student's arrival to campus. 

Sickle Cell Information

  • NCAA policy dictates that all student-athletes new to an institution must provide proof of their sickle cell status
  • Most people born in the United States were testing at birth; instructions on how to request your records vary based on what state you were born in. 
  • Students who cannot get their results will need to obtain testing. This can be completed with your primary care provider. The needed test is called hemoglobin solubility or hemoglobin iontophoresis test.
  • Results can be faxed to Millersville University Athletic training at 717-871-7885. Please ensure the student-athlete’s name and M-number are on the results.
