  • What immunizations are required?

    For residential students there are three immunizations that are required:

    1. A Tetanus (update within the last 10 years),
    2. A record of two Measles, Mumps and Rubella Immunizations or positive titers, and
    3. A Meningitis Immunization or a signed wavier. 

    For non-residential students there are two immunizations that are required:

    1. A Tetanus (update within the last 10 years) and
    2. A record of two Measles, Mumps, and Rubella Immunizations or positive titers

    Please check with your department for entrance requirements for your major.

  • What if I can't find my immunization records?

    Many students check with their family practitioners for immunization records. You may also be able to obtain your records from your parents or your school district. If you are still unable to find your records, it is recommended that you receive a Tetanus booster and have Measles, Mumps, and Rubella titers drawn to meet the requirements.

  • Who may complete the Health Evaluation form?

    There are two sections to the Health Evaluation forms, one that you the student must complete and one that your family practitioner must complete. Your family practitioner can be a physician, a nurse practitioner or a physician assistant. Please make sure they sign the form and include their license number.

    You can drop off, mail, e-mail or fax your completed forms to Health Services.

  • I am a commuter. Do I still have to complete the Health Evaluation forms?

    Yes. The Health Evaluations forms are required for every Millersville student. These forms provide us with invaluable information that is used to provide medical care, especially in emergency situations.

    Yes. If you plan to use Health Services, the Health Evaluations forms are required for graduate students. These forms provide us with invaluable information that is used to provide medical care.
