SMC Event Setup & IT Request Form

Timeframe:  This form should be submitted at least two weeks in advance of the scheduled meeting/event.  Requests made within less than 5 business days before the scheduled event cannot be guaranteed and are subject to the availability of staff and facility resources at the time.  

This form is only for events scheduled in the Student Memorial Center (outdoor/indoor), the SMC Rec Center (Ville Courts), and the Quad.

This form is for faculty and staff to request customized space setup, additional event resources, and Audio-Visual requests for spaces managed by SMC Operations and/or Campus Recreation. Please ensure you have reserved the space through Ad Astra before completing.
Student Organizations should NOT use this form and instead submit all event information through the Event Registration Process in Get Involved.
