Financial aid Loan Change Request Form


Do you need to make changes to your offered or existing student loans?

Whether you're adjusting your Federal Direct Subsidized or Unsubsidized Loan, Federal Direct Graduate PLUS Loan, or Alternative/Private Loan, the Loan Change Request Form is the tool you'll need.

If you are a parent and need to make a change to your Federal Direct Parent PLUS Loan, please reach out to our office @ fa.mail@millersville.edu

Access:  Available to confirmed Millersville University students through StudentForms.
How to Complete:

Log in to Student Forms or create an account. You need to create an account every academic year. Then submit the Change of Enrollment Form before the start of the semester. Make sure to estimate your enrollment plans for the entire academic year. 

  • Click Manage Requests in your Student Forms account.
  • Select the Loan Change Request Form.
  • Complete, sign, and submit the information.
  • Wait for the form to be reviewed and processed.

Just need to accept or decline your loans?

The fastest way is through your Millersville MAX account. Once you complete the process, your financial aid awards will automatically update, provided that your financial aid requirements are complete.

Having issues with MAX? 

If you're unable to complete the update through MAX, you can use the Loan Change Request Form as an alternative.
