Financial Aid Authorization to Release Information
If you’d like a parent, guardian, spouse, or someone else to speak with the Office of Financial Aid on your behalf, you must complete the Authorization to Release Information Form.
What the Form Does
- Allows Financial Aid to discuss your financial aid details (awards, status, required docs) with the person you choose.
- Does NOT cover grades, class registration, academic progress, or full billing info.
- Optional release of billing details available
Who can complete this form
- Confirmed Millersville students with an MU login
- Unconfirmed students—don't worry—we can discuss your account without this form up until you commit to MU
How to Complete the Form
- Go to Student Forms
- Log in OR create an account — you need to set up your account every year!
- Click Manage Requests
- Select Authorization to Release Information
- Fill it out, sign, and submit
- Allow 2-5 business days for processing — Your Financial Aid Overview in MAX will show this requirement as satisfied
Why This Is Required
You aren't required to complete the form itself, but if you do not, we cannot discuss your account with anyone else.
Under the Family Educational Rights and Privacy Act (FERPA), we cannot share your financial aid details with anyone unless you give written permission through this form.
How Long Is It Valid?
- Authorization remains valid for the life of your student record
- You can revoke or update it anytime by contacting Financial Aid in writing
