Financial Aid Authorization to Release Information
If you want to authorize a parent, guardian, spouse, or another third party to speak with the Office of Financial Aid on your behalf, you must complete the Authorization to Release Information Form.
This form allows us to share specific details about your financial aid record with individuals you name—only after your written consent is received.
What You Need to Know
Who Completes This Form | Confirmed, Millersville students who wish to grant access to their financial aid information. |
What This Form Does | By completing this form, you give the Millersville University Office of Financial Aid permission to discuss your financial aid status, awards, requirements, and documentation with a designated third party. |
What This Form Does Not Do |
This form does not grant access to academic records or student billing details. For billing information, grant third-party access through the Office of Student Accounts. For academic information, contact the Registrar.

Why It's Required | Under the Family Educational Rights and Privacy Act (FERPA), we are legally prohibited from sharing student-specific information with anyone other than the student unless a release form is on file. |
How Long It's Valid | Authorization is valid for the life of your records—however, you can revoke authorization at any time by notifying the Office of Financial Aid in writing. |

Complete the Form | Fill in all required fields, including student and authorized individual information. Don’t forget to sign and date the form. |
How to Submit | If completing a paper form, return it to the Office of Financial Aid in person on the second floor of Lyle Hall or via email. |
